A man has been seriously injured after stopping at a road rage dispute on a busy Perth highway where police allege a driver pulled out a knife.

Authorities say a verbal altercation erupted when the drivers of a Hino tow truck and a Fiat Ducato got out of their cars on Great Eastern Hwy in the inner-eastern suburb of Ascot about 12.50pm on Wednesday.

The showdown escalated when the tow truck driver allegedly “brandished a knife in an aggressive manner”.

Police said a man who had stopped at the scene was injured when he was struck by the tow truck as it was driven from the area.

Phone vision of the incident showed the man, aged in his 30s, writhing in pain in the middle of the highway.

He was taken to Royal Perth Hospital by St John WA with serious leg and pelvic injuries and remains in the care of doctors.

The incident blocked traffic for several hours as police investigated what happened.

Tow truck driver Dylan James Porch, 44, was charged with being armed in a way that may cause fear and unlawful actor or omission with intent to harm.

The accused, from Ellenbrook, made his first appearance in Perth Magistrates Court on Thursday and remains in police custody.
